The White Oak Foundation will serve as a COVID-19 “Rapid Response” Test Site Saturday, Sept. 12,  9 a.m. – 2 p.m.

1. COVID-19 “rapid response” testing will be administered by Goshen Medical Center. Testing time to notification: @15-20 minutes.

2. You must pre-register. Registration forms are available in the Foundation Office; or can be printed from the Website: (www.whiteoakfoundationnc.org). Upon request, the forms can be emailed to you: (juanita.young31@yahoo.com).

3. All forms must be completed and signed by each patient or a guardian. If multiple members of the family are to be tested, you must have separate, signed registration forms. Minor children must also register.

4. To register, call Janet Burnette (919) 362-6799 (Ext. 300) to receive an appointment time which is to be printed in the upper right corner of the registration form. Insurance cards should also be attached to the application.

Remember: There are no out-of-pocket costs.

5. On test day, please have completed registration forms available for medical personnel as you drive through. The telephone number on your form is the number by which you will be notified of your test results.

6. To avoid traffic hazards/congestion, it is suggested that you NOT arrive more than 5 minutes ahead of your appointment time.
